§ 6-124. Issuance and display of certificate; notification of changes.

(a)

The director shall issue a certificate to the person paying for a commercial pet service facility license, which certificate shall contain the information specified in section 6-125 of this Code. The licenses shall display the certificate at all times in a prominent place in the commercial pet service facility.

(b)

A licensee shall notify the director by personal delivery or certified mail of any change in the name, address, management, or substantial control or ownership of ten percent or more of the outstanding shares of stock as shown on the license application of his business or operation within ten calendar days of any such change.

(c)

A license issued under this division shall be personal to the applicant and shall not be transferrable or otherwise assigned.

(d)

A license issued under this division shall only be valid for the address of the facility provided in the application.
